Hua Quing Ruan, a forklift operator, was killed when the forklift he was operating tipped over at the bottom of a ramp leading from the company's loading dock. Ruan was descending the ramp with a bag of mortar mix on a wooden pallet to load into a customer's car. The bag fell off and blocked the front wheel of the forklift. Ruan attempted to back up to free the wheel, but the forklift rolled off he ramp. Ruan attempted to jump to safety as the forklift began tilting over, but he was hit by the forklift and crushed. He was pronounced dead at the scene. According to some sources, Ruan was not certified to operate the forklift and was not wearing a seatbelt. The ramp he was operating the forklift on was not up to code because it did not have a curb to prevent the forklift from rolling off of the side.
